  FBI Clearance

• Registration Required Prior to Starting Service
  Fingerprinting and Report required within two weeks of start date.
   $33.00 plus $2.50 member copy

  Register Online
  Select Intermediate Unit under Reason for Request
  Enter Required Information
  Check box for personal copy if desired (Recommended)
  Print 2 Transaction Receipts for Reimbursement and Fingerprinting
  Review Fingerprinting Site Locations and Hours (some sites require appointments)
  Bring registration and photo ID to site for fingerprinting scan
  Provide PAE Registration ID (alpha and numeric characters) to PMSC to obtain FBI Report
  Retain Copies for reimbursement and records

  Child Abuse Clearance

• Required within 90 days of starting service
  $10.00 money order
  Complete paper Child Abuse Form

  Check “School” under “Purpose of Clearance”
  Use your name and address as Requester
  Obtain a money order for $10
  Retain original money order receipt for reimbursement
  Copy completed form for your records
  Submit form and payment to address indicated
  Show original clearance report and one copy to PMSC when received
